Are Anthy and Akio related?
Akio Ohtori Akio is the interim chairman of Ohtori Academy and the older brother of Anthy. At first, Anthy appears to simply be Akio’s loving and proud sister. However, her relationship with him is much more complex as Akio is controlling, manipulative and abusive towards Anthy.
What does Akio do with Anthy?
The Movie. A flashback shows that Akio slipped a drug into his sister Anthy’s drink and then took advantage of her sexually. This is icky enough, but it then goes on to show that afterward he realized that she was awake and began to panic, and wound up stabbing her and then falling out a window to his death.

It is possible that Anthy and Akio chose their their last names since they don’t have real family names being gods and all. Also their names are different for symbolic reasons, Ohtori means Phoenix while Himemiya means Princess Shrine.
Why did Anthy stab Utena?
Anthy stabs her, for the reason that because Utena is a girl, she could never be Anthy’s prince. And in the wreckage of the dueling arena, Anthy/the Rose Bride ascends and is re-consumed by the swords of yore.

What happens to Utena?
Behind the Gate is Anthy, inside a coffin, who pleads for Utena to not try to save her. Utena doesn’t listen and begs Anthy to take her hand. Upon doing so, the coffin breaks off of the arena and Utena is pierced by the swords in Anthy’s place, which then destroy the arena.
